Summer activities are in full swing now, and as your pediatricians in East Portland, we know that means that sports forms and physicals are often due right away, especially for high school students participating in summer practices. If you’re uncertain what a sports physical entails and if your child needs one, you’ve come to the right place. Here we’ll break down how our clinic handles both and what next steps you should take.

Full Physical vs Sports Physical

The good news is that you DO NOT have to make a separate appointment for your child to have a sports physical if they’ve had a full physical in the last 12 months. They’re good to go! This is because we do a full physical at every annual well child visit, and a sports physical is included in that.

Annual Checkups are Important

Your child’s annual checkup is much more important than a basic sports physical. Not only do we address all your health concerns, we look to see if your child’s vision or hearing are due for testing, check in on their mental health, immunization records, medication refills and more.

Where to Find Our Sports Forms

If your child is an East Portland Pediatric Clinic patient, the first step is to check your portal. You may already have a current sports physical form already on there; most schools accept a sports physical form from the last 2 years. If you don’t see one and your child has had a physical in the last year, here are the links to fill out our sports forms for Oregon and Washington.
