COVID-19 Precautions
Call us on arrival if you have been exposed to Covid-19, tested positive for Covid-19 in the last 10 days, or have any of the following symptoms: cough, fever, sore throat, runny nose, frequent sneezing, headache, vomiting, diarrhea, or loss of smell. We may need to direct you to a different entrance or reschedule the appointment.
At this time the only entry is through the main Pavilion entrance, which faces Main Street. Use the West elevators and come down one floor.
The west pavilion doors will be locked on all weekends & holidays. You should enter through the main hospital entrance on 100th & Market & follow the signs to the pavilion. Then take the west elevators to the LL. If you are instructed to meet our staff at the side entrance and you are unable to use the stairs or are using a stroller, please let us know and we will give you another option.
A surgical mask must be worn properly at all times while in the facility, even if you are vaccinated or in an exam room. Cloth masks are not allowed. If you don't have a surgical mask, we will provide one.
While masks are not required under age 5, they are recommended down to age 2. Let us know if there is a physical or developmental reason a mask can not be worn so we can arrange an accommodation.
Please bring your current insurance card.
Call to cancel or reschedule 24 hours in advance to avoid a missed appointment charge.
