A Smarter, Easier Way to Prep for Your Child’s Check-Up: Meet CHADIS

We are always looking for ways to make your experience at our clinic as easy as possible. Starting January 7th, we’re excited to introduce our new online screening tool: CHADIS!

A secure, easy-to-use online tool, CHADIS is designed to help you provide important information before your appointment. So instead of rushing to fill out paperwork in the waiting room just before your appointment, you can complete it all beforehand at home and on your own schedule.

CHADIS questionnaires will start in early January 2026.

 

Why Use CHADIS?

CHADIS helps us get a more complete picture of your child’s health by collecting important information and your concerns before your visit. The questionnaires help us identify early signs of developmental delays, behavioral concerns, social or emotional challenges and other health risks that might not be obvious during a typical appointment. This allows your pediatrician to focus on what matters most to you during your visit and providing the best care possible.

By using CHADIS, you’re helping us come to your child’s visit prepared and informed. We’ll review your responses during the appointment and address any questions or concerns you may have. This saves time at check-in and gives you more quality time with your pediatrician.

 

How Does It Work?

Before each scheduled well-care visit, you’ll receive an email or text prompting you to log in to your patient portal. From there, just click the orange “Start Questionnaires” button to access CHADIS and complete the assigned forms. It’s that easy!

 

 

Clicking the orange button opens the questionnaire on CHADIS’S website in a new browser window. There may be multiple questionnaires to complete, depending on the patient’s age. Click to open a questionnaire and begin filling it out. When one is completed, it will automatically take you to next one. You can complete these on a computer or on your phone, whichever you prefer. 

To use CHADIS, you’ll need portal access at home. If you haven’t set that up yet, just give our office a call and we’ll walk you through it. And for our teen patients: it’s important to set up your own portal account to ensure privacy and give you the chance to share your health concerns directly with your provider.
